Please complete the form below to request benevolence assistance on behalf on an individual. This form is to be completed by the Staff Member, Deacon, Small Group Leader or Board Member that is making the request. It is not to be given to the individual that assistance is being requested for. All payments will be made directly to the vendor to which the payment is due.
